   The reason Goku's symptoms showed up much later than future Goku's is because he didn't go Super Saiyan and fight Freeza and King Cold. Trunks specifically told him to take it when the symptoms pop up because its a CURE not a vaccine. Taking it before getting sick would be a waste of the medicine and extremely dangerous.

      Another reason Trunks fighting Freeza and King Cold screwed things up. I also theorize that the reason the Androids underestimate Goku is because he never fought Freeza and King Cold. Otherwise, he would have released 17-18 first instead but would have nerfed them in attempt to control them better. Which is why Trunks later says the Androids in this timeline are stronger.

      Yeah, this is the biggest problem people have with the writing. Tien noticed first that Goku had trouble and 10 seconds later he is surprised by it. Gohan and Piccolo are 2 of the smartest characters and they forget so much stuff, just like here with the cure. Which I guess they told Gohan about the heart disease.
