On the wearing heels thing, it's actually very common to put an actress in heels if they feel she's too short. Freema Agyeman is apparently 5'2, while Tennant is 6'1. For comparison, Billy Piper is 5'5, and Tennant towers over her. While obviously heels are often used to make an actress "sexier", I'd put money on height being the reason in this case, though I don't remember if Martha wears them much after this episode.

Yknow Ruff they had slaves in England before America. Doctor Who tries to do a lot of British history re-writing. They were the first to outlaw slavery, but in 1599, it was not like as depicted.
The fun thing about TV and movies when they do a shaking scene can be seen really well in the movie Galaxy Quest. First, they show it the way it's usually done, with the actors just sort of throwing themselves about. But then later on they actually have the set on a gimbal so that the actors are really being thrown around and it goes to show the difference visually. Really great.
I don't know if they figured it out in the stream but I think the thing she knew the actress from was the movie what a girl wants
